Only cars i've had to have anything reflash is Ford since they sometimes lock the BCM.

Your EVO-ALL though right now is flashed with as a GMC Canyon : http://fortin.ca/en/support/001a06292056/

(also, there is no options for 3x LOCK start set up if you intended on using the unit as a 3x Lock starter)

Honestly, it's actually the same firmware so it should not affect programming at all. It just leads me to believe that module may have been used previously on another module and that you may just need to do a master reset.

Master Reset Procedure:

  1. Hold down programming button while plugging in the datalink connector.
  2. Let go of button when LED is RED.
  3. Push and hold button again until all three LEDs start to alternate.
  4. Disconnect unit, she is now reset.

Do that master reset and try again. If it does not program again, give me feedback... where does the programming stop (which step, and what do the LEDs on the module do)? are you using a T-harness? etc...
